We will help you fill out the paperwork, and then we’ll take care of the rest.
You Could Also:
- Visit your current hearing care provider in person to ask them for your records. You need copies of your latest hearing test, and records of your current hearing aids.
- You can also print this form, complete it and take it to your current hearing care provider. Be sure to check that all details are complete before taking it in to them. They should fax the information over to us ASAP.
You can also verbally request to be taken off any mailing lists or marketing lists if you wish at that time to reduce the amount of junk mail you receive (we promise to limit any mail we send you to an absolute minimum!)
